Assumption of Risk
By registering my child, I understand that I am giving my consent for participation by my son/daughter. I understand that tumbling, cheerleading, and Ninja Warrior Training are all activities which may involve airborne inversion of the body and therefore there is an increased potential that any one of the activities involving my son/daughter's participation could lead to serious injury, paralysis or even death. I understand that our son/daughter is required to be in good physical shape and condition and that the activities in which he/she will be asked to participate are strenuous and require physical and athletic agility. It has been fully explained to me that these activities include, but are not necessarily limited to a variety of gymnastics maneuvers, including somersaults, back handsprings and aerials; that there will be a variety of mounts, tosses, and stunts requiring the coordination of more than one participant on the squad; and that these activities will not be confined to any one site or venue, but rather will involve a variety of sites or venues throughout the year. I represent to you that, to the best of my knowledge and belief, my son/daughter has no physical, medical, or mental disability or other limitation that would restrict his/her ability to fully participate in this activity. I further agree to notify immediately the appropriate personnel in the event of any change in our son/daughter's health status.
Release of Liability
I agree to, and by signing this agreement, release the coaches, advisors, volunteers, other students, and staff of Anointed Athletics from any claim of negligence by ourselves, our son/daughter, our heirs, executors and assigns, from any liability arising from claims for damages for injury to my son/daughter and any claims for loss of or damage to his/her property which may arise out of his/her participation in program.
Medical Emergencies
My child has my permission to participate at Anointed Athletics. I confirm that my child is in good health. I am also fully aware of and appreciate the risk of participating in any activity on the premises of Anointed Athletics including personal damages and losses; accidental injuries, minor or serious, including head
and neck injury; and loss of life. I hereby give my permission for Anointed Athletics officials to call a doctor and/or paramedic for treatment in the event of an emergency. I further agree not to hold any Anointed Athletics official, staff member, or athlete responsible for any possible illness, accident or injury which may occur in training, class, on Anointed Athletics premises, or during events at which Anointed Athletics may participate or perform.
Infectious Disease Clause
Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
Dress Code
NO JEANS IN CLASS?EVER!!!! Students will be tumbling, exercising, or stunting and
that is very difficult to do in jeans. Spare clothes may be left with the coaches if necessary
to prevent this. Any athlete wearing jeans to practice may be asked to sit out.
No Jewelry of any kind. Wearing jewelry during tumbling can result in injury.
Bodies must be covered modestly at all times. For girls this means that you may not
come to practice in only a sports bra and shorts. And for boys this means shirts may not
be removed.
During Anointed Athletics events, this means being dressed correctly in the predetermined attire for that event.
Refund Policy
Anointed Athletics requires a 30 day written notice if your child will be leaving the
program. This is so there is no misunderstanding of whether or not your child has decided to
leave the program, or is just missing due to illness or other commitments.
ANY CHILD ABSENT FOR THREE CONSECUTIVE PRACITICES OR MORE WITH NO
NOTICE MAY BE REMOVED FROM THE ROSTER AND REPLACED WITH SOMEONE
ON THE WAITING LIST. ANYONE REMOVED FROM THE ROSTER IS ONLY ENTITLED
TO REUTRN IF SPACE IS AVAILABLE.
No refunds are given for monthly fees.
Code of Conduct
No profanity or abusive language
Absolutely NO bullying, mocking, or saying of rude things to another person-this
includes our team members, members of opposing teams, other children, any and all
parents and adults. EVERYONE will be respected AT ALL TIMES.
FACEBOOK AND SOCIAL MEDIA WEBSITES ARE TREATED AS IF SPOKEN IN
PERSON TO YOUR COACH AND YOU ARE THEREFORE RESPONSIBLE FOR
ANYTHING SAID ON THESE. INAPPORPRIATE BEHAVIOR OF ANY KIND SUCH AS
TRASHTALKING, BAD MOUTHING, DISRESPECTING TEAMMATES, COACHES,
OTHER PROGRAMS OR ANOINTED ATHLETICS WILL NOT BE TOLERATED
No cell phones during practice or performances. If parents need to be contacted, the
coaches can do so.
Athletes are required to pick up after themselves. This is a rented facility and we will
respect it!
Payment Policy
Payments are due the first of every month.
Payments received after the 10th day of every month is subject to a $30 late fee.
Any student not current after the 15th day of each month is subject to being dropped from
the class.
Any returned check/payment is liable to a $50 NSF fee that must be paid in cash along with initial payment before child will be allowed to resume classes.
