Welcome to MAX Movement! We are glad to have you join our dance/gymnastics family! Please complete the following registration form. *We have an enrollment/registration fee of $25 for new students and $20 for returning students. We do offer family discounts on the registration fees. **There is a section below to enter card information, but it is not a required field. This section will store your card info if you are interested in making online or automatic payments.

**Gymnastics classes will need to be selected by teacher.  Gymnastics classes are determined by skill level.  Please leave the class selection blank, fill out the remainder of the form, submit it, and then contact us via email (maxmovementdancefitness@outlook.com), via Facebook message, or via phone (423)237-3823 so that we can select the correct gymnastics class for you.
