Studio Policies
2021-2022 PARENT AND STUDENT TERMS OF ENROLLMENT, RELEASE AND WAIVER
This Parent and Student Terms of Enrollment, Release and Waiver Document ("Terms of Enrollment") is executed on the date below by and the undersigned Parent and Student (or if Student is not 18 years of age or older, the Students' parent or legal guardian on behalf of Student). The undersigned Parent and Student agree to abide by each and every one of the following terms in order for Student to receive dance instruction from NorthPointe Dance Academy, LLC, a limited liability company ("NorthPointe") and to participate in the related activities as a part of such enrollment. For the 2021-2022 enrollment term all of the terms below are subject to modification in light of the COVID-19 pandemic. Any such modifications including the Safety Measures as defined later in this document will be communicated in writing to the Student and Parent. NorthPointe will present a formal dance recital at the end of the 10 month program, date TBD. The recital is an excellent opportunity for Student and all students to display all of their hard work to friends and family members. However, participation in the dance recital is not mandatory. Student should arrive to class promptly. Student's hair is to be pulled away from face and secured neatly and no jewelry is permitted in class. Student should have a dance bag that will hold all shoes and Student should bring that bag to every class. Student should take the bag into class with them instead of leaving it in the hallways. Proper class placement is essential. Parent agrees to support NorthPointe decisions and selection of Student's class placement. NorthPointe reserves the right to cancel any classes with 7 students or less. Parent understands that Students, siblings, and visitors under 13 must have adult supervision at all times while in our facility. Parent agrees to never leave a child/or Student (under age 13) unattended unless they are in class. NorthPointe may close the studio due to weather emergencies or for other unforeseen reasons, including an ordered closure. Cancellation of daytime classes will be determined by 8:30 am and late afternoon/evening classes by 2:00 pm. Students will be provided the opportunity to make-up these classes. If there is any interruption to in-studio physical dance instruction, (beyond the normal, brief weather-related incidents) NorthPointe will immediately transition to the NorthPointe Digital Dance Connection Series Video Library and Live Stream Classes. Food and drink are permitted in the Barre eating area only. Water bottles only are permitted in the classrooms. Please help maintain our beautiful facility by placing items in trash receptacles. Absolutely no chewing gum in the studio! There are observation windows that will allow Parent to observe Student in recital class. Family and friends are invited to attend class with Student during late fall and spring family nights. NorthPointe is not responsible for lost personal items. Parent and Student understand that NorthPointe will not maintain a lost and found and any items remaining at the end of the business day's close at the Northpointe studio will be discarded. NorthPointe does not and will not accept any liability for any lost or stolen items of the Parent or Student. Part of your commitment as a parent is to know where to find information. We work hard to make information available and readily accessible to each of our parents. Your weekly routine should include checking our NorthPointe website, Facebook page and checking any emails or text messages that you may have received from us. Parent will only discuss Student's progress and not other student's progress at the school with the Directors and Staff of NorthPointe. Parent will not compare Student to other dancers and will be supportive and nurturing to Student and all NorthPointe student's development in dance. A positive attitude and continued support is necessary in order for Student to continue with the privilege of participating in the NorthPointe's program. Parent and Student agree that if they choose to have a negative attitude or violate the terms of this Agreement in any way that, at the sole discretion of NorthPointe, Student and Parent may be asked to leave the school without refund of any fees or deposits paid. NorthPointe reserves the right to refuse services and/or discontinue services to any person who does not support the policies and decisions of NorthPointe.
2021 Summer Camp Tuition Policies
This Agreement ("Agreement") is made on this date by and between North Pointe Dance Academy, LLC, an Ohio limited liability company (hereinafter “NorthPointe”), and the undersigned Parent and Student. The undersigned Parent and/or Guardian of the student, for valuable consideration received, agrees as follows:
Parent will pay a Summer Camp registration fee of $40 per dancer. Parent understands that this fee is due in full at the time of registration and is non-refundable. $25 of the registration fee will be credited to my summer camp fees when they are charged, 10 days prior to camp. Registration fees are non-refundable and are credited only to 2021 Summer Camps. Once Summer Camp fees are charged, Parent understands that these fees are non-refundable.
2021-2022 Fall Session Tuition/Fees Policies
This Agreement ("Agreement") is made on this date by and between North Pointe Dance Academy, LLC, an Ohio limited liability company (hereinafter "NorthPointe"), and the undersigned Parent and Student. The undersigned Parent and/or Guardian of the student, for valuable consideration received, agrees as follows:
Parent accepts full responsibility for all payments due to NorthPointe as a result of Student's enrollment and participation in NorthPointe's dance academy. Student's tuition at North Pointe is due on the seventeenth of each month. Class fees have been divided into 10 equal monthly payments, beginning August 15th, 2021 and ending on May 15th, 2022. If tuition has not been paid by close of business on 20th of each month, a $25.00 late fee will be automatically debited from Parent's account, along with tuition. If tuition has not been paid by the final business day of the month, Student will not be allowed to take class. Parent understands that the tuition check may be mailed to the school prior to the tuition due date, if it cannot be personally delivered. Parent understands and agrees that any tuition payment made is non-refundable. It is the policy of NorthPointe to automatically deduct Student's tuition fees from a credit or debit card that Parent will provide. At least thirty days written notification by Parent to NorthPointe that Student will no longer be attending NorthPointe is necessary in order to stop charges to Parent's account. Parent will pay a yearly registration fee of $40 per dancer and $15 for each additional family member. First month's tuition is required at the time of registration, beginning August 15th, 2021. Parent understands that this fee is due in full at the time of registration and is non-refundable. To ensure full enjoyment and continued growth in dance, Student's regular attendance is essential. If notification is received prior to missing a class, a make-up class may be arranged. Make-up classes must be completed within 30 days. MISSED CLASSES ARE NOT REFUNDED. There will be NO credit for missed classes. Students are able to make-up a missed class with a similar type and level of class. NorthPointe is unable to provide make-up classes after April 15th, annually, due to recital preparation. There will be no make-up classes offered for scheduled holiday closings. Please refer to our Academy Calendar for these dates. Prorating fees is not allowed unless Student is a new student beginning in the middle of the month. Parent understands that NorthPointe offers a generous make-up class policy. Please contact our front desk staff to schedule any make-ups. Costume fees for the recital will be $85.00 plus tax ($90.95 total) for all Creative Classes and younger. All other classes will have a costume fee of $90.00 plus tax ($96.30 total). Parent agrees that these costume fees must be paid in full by November 1st, 2021. Any recital costume ordered on or after December 15th, 2021 will be charged an additional $10.00, plus tax. Parent agrees that these costume fees are non-refundable. Costume fees do not include any costume accessories such as tights, gloves, shoes, etc. Any recital costumes not picked up by the final day of classes for that season, will be donated. A recital fee of $50.00 for the first dancer and $25.00 per each additional dancer will be paid to NorthPointe by April 1, 2022. This fee is in lieu of purchasing individual tickets for the performance.

I hereby grant NorthPointe Dance Academy permission to take photographs and/or videos of my child (NorthPointe students), to use in brochures, web sites, posters, newspapers, advertisements and other promotional materials the schools create. Permission is also hereby granted for NorthPointe Dance Academy to copyright such photographs in its name.
Release and Liability Waiver
ACKNOWLEDGEMENT OF AND CONSENT TO PARTICIPATE IN STUDIO HELD DANCE CLASSES AND ADHERE TO SAFETY MEASURES:
Parent and Student hereby authorize NorthPointe its managers, members, staff, employees, agents, representatives and independent contractors to instruct my student in the art of dance and related activities. I recognize the inherent dangers associated with such activities, including, but not limited to, falling, twisting, breaking, and/or injuring limbs, hips, knees, elbows, neck, head, paralysis and related injuries. Parent and Student also understand that participating in group activities may result in exposure to contagious diseases and illnesses, and such exposure may not be completely preventable. Parent and Student understand that while NorthPointe will take reasonable care in providing instruction, Parent and Student agree and accept the fact that there are numerous injuries which may occur in the course of that instruction. Parent and Student further understand that while NorthPointe is taking reasonable care with its studio facility by incorporating and enforcing its Safety Measures, the exposure to contagious disease and illness is often unpredictable and not completely preventable. Accordingly, Parent and Student hereby release and waive any claims that the undersigned may now or hereafter have and absolve NorthPointe, its managers, members, staff, employees, agents, representatives and independent contractors from any and all claims and causes of action arising out of their care and instruction of Parent and/or Student, including, but not limited to, all liabilities, losses, damages, costs or expenses of whatever kind or nature, including attorney's fees, which may incur as a result of any injury or illness, including, without limitation, personal injury, personal sickness and personal illness and damages thereof including loss of income, earnings, bodily injury, pain and suffering, emotional or mental distress and any and all medical expenses. Parent and Student further agree that there shall be no refunds for tuition, costume fees, performance fees, registration fees, convention fees, competition fees, or private lessons.
Parent and Student each consents to Student participating in studio classes during the COVID-19 pandemic. In light of the Pandemic, NorthPointe has incorporated into its business operations various safety measures for the safety of its students, their families and its staff. These safety measures are based on recommendations from, but not limited to, the following sources: the Centers for Disease Control and Prevention and the Ohio Department of Health as may be later modified, collectively referred to as the Safety Measures. These Safety Measures will be in effect immediately, will continue to be in place until further notice, and will require that Parent and Student adhere to and comply with them.
The undersigned Student (if 18 years or older), or the Student's parent or legal guardian, acknowledge the following:
Parent and Student consent to Student participating in dance classes as offered by NorthPointe during the Pandemic and are aware and acknowledge that there is a risk of transmission of the COVID-19 virus and they each assume that risk. Parent and Student further acknowledge that they have received a copy of, have read, understand and fully agree to abide by and adhere to the Safety Measures. Parent and Student each further acknowledge that they have each signed this Terms of Enrollment voluntarily and of their own free will in exchange for the privilege of Student enrolling and participating in dance classes at NorthPointe.
